Based on observation, the licensee did not comply with the section cited above in 4 outdoor solid waste containers located on preschool playground were observed without lids which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview, the licensee did not comply with the section cited above in facility does not have a carbon monoxide detector in the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

(b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidence by: Based on results submitted to the Department, the facility did not comply with the section cited above in the water outlet identified as "A" tested above 5.0 ppb on 12/2/2022. Outlet "A" was reported to have an exceedance of 113 ppb, exceeding allowable limit for lead which poses a potential Health, Safety or Personal Rights risk to persons in care.
