Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in licensee did not have a current roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will provide proof of completion to LPA via email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in fire drill was not completed every 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will provide proof of completion to LPA via email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in licensee did not document 15 minte checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will provide proof of completion to LPA via email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in Licensees did not have current MMR, Tdap, Tuberculosis which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will provide proof of completion to LPA via email.

Based on observation and interview, the licensee did not comply with the section cited above in by having 1 adult working at facility without criminal clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have adult complete and clear a criminal background check before being allowed to return to the facility. Licensee agrees to submit a copy of receipt of completion of livescan of individual .

Based on observation and record review, the licensee did not comply with the section cited above in 3 out of 3 infants not having a complete LIC 9227, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete have all 3 infants complete an LIC 9227 and will email LPA forms by May 13, 2022.

Based on observation, the licensee did not comply with the section cited above by having 1 out of 3infant placed in a baby bouncer which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed baby bouncer from facility and will inform parent the items is not allowed. Licensee will reviewed prohibited items in Family childcare homes and complete a declaration and email LPA by 5/13/2022.
