Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in 1 adult did not obtain fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure adult is fingerprinted and will follow up with the Rgional Office to ensure adult is fingerprint cleared. Licensee will watch the "background check requirements for caregivers" video on ccld.childcarevideos.org and submit a summary to LPA via email

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in licensee was caring for 5 infants at once, LPA observed 1 infant was picked up before nap time, putting her back in ratio, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will watch the "how many children can attend a family child care home" video on ccld.childcarevideos.org and submit a summary to LPA via email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above, the backyard was observed to have multiple areas that put children at risk such as loose bricks , gardening tools, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure children do not have access to the off limit areas by making the dangerous areas inacessible to children in care/ removing dangerous items from backyard.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in 2 adults had expired mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to make sure trainings are completed and a copy of the certificate is sent to LPA via email.
