Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in facility did not have a working carbon monoxide which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide LPA a video email showing the working Carbon Moxide in both the home and the detached activity room.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in LPA observed a substance under the unlatched/no baby gate kitchen sink which was "pesticide free" rodent repellent. LPA also observed a a knife in a cubby in the activity room that was at least 4.5 feet high which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Ornelas observed licensee remove the substance from under the sink and place in an outdoor storage shed. LPA also observed licensee remove the knife from the activity room.
