Based on record review, the licensee did not comply with the section cited above due to was not able to locate the children's files with the CDPH 286 Immunization Record form which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will request all of the children's immunization records from parents again, and complete the CDPH 286 Immunization Record form, if can not locate the children's records. Licensee stated she will submit proof of immunization records to LPA Rosales-Meza via email: leticia.rosales@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above due to the licensee was not able to locate the children's files with the LIC 995A, LIC 995E, and LIC 9212 forms which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will produce the LIC 995A, LIC 995E and LIC 9212 forms for each child in care, if can not locate children's records and have parent sign and date the bottom portion of the notice form LIC 995A. Licensee stated she will submit proof of records to LPA Rosales-Meza via email: leticia.rosales@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above due to the licensee was not able to locate the children's files with the required forms which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will reproduce all children's forms and have parents sign and date all of the required forms needed for each child enrolled. Licensee stated she will submit proof of records to LPA Rosales-Meza via email: leticia.rosales@dss.ca.gov.

Based on interview and record review, the licensee did not comply with the section cited above, the licensee was not able to locate the children's files with the signed LIC 9150 form which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will produce the LIC 9150 form each child in care, if can not locate children's records and have parent sign and date the bottom portion of the notice form LIC 9150. Based on observation, interview, and record review, the licensee did not comply with the section cited above, the licensee was not able to produce a current roster, form LIC 9040, missed placed it which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will reproduce and complete a new facility roster (LIC 9040) and send a copy to LPA via email: leticia.rosales@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above due to the licensee was not able to locate the children's files with the LIC 995A, LIC 995E, and LIC 9212 forms which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee not furnishing Sleep Logs or evidence to prove that 15-minute checks had been conducted for C1, C2 & C3. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
