Based on observation at 2:15, LPAs observed a bottle of weed killer on the ground in the backyard which was accessible to children by an unlocked door in the den; at 2:39pm, a bottle of bleach on the floor in the laundry room that was accessible to children by an open door; and at 2:51 a can of lysol and bottle of windex were observed pushed back on a table but accessible to children in the front outdoor play area, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Submit a written and signed statement going over where poisons and cleaning chemicals are to be stored with the adults in the home and will review the plan with the adults in the home. Statement can be sent to Dianne.morrison@dss.ca.gov.

Based on observation at 1:31 pm Infant (C1) was observed asleep on the couch, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Submit a written and signed statement of the understanding that infants must be placed in a play yard or crib to sleep, or as soon as possible after falling asleep.

Based on record review, the licensee did not comply with the section cited above in three out of four children's records. L1 did not have immunization records available for C1-C3 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Obtain immunization records for children C1 - C3 and provide proof of immunizations and CDPH 286 to LPA Morrison by 12/30/25.

Based on record review, C4 was missing LIC627. Based on interview the licensee did not comply with the section cited above. L1 did not have records for children C1-C3, including LIC700, LIC995A, LIC627, LIC282, and LIC9150 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Obtain all required forms from parents and file them into individual files and have on file for review.

Based on interview, the licensee did not comply with the section cited above in that L1 did not have form LIC9227 on record for review for one infant, C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain form LIC9227 from C2's authorized representative and place in C2's file. Licensee will submit form LIC9227 to LPA Morrison. Email: dianne.morrison@dss.ca.gov

Based on interview, the licensee did not comply with the section cited above in 2 out of 2 infant files. L1 did not have sleep logs for infants C1 and C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will begin logging sleep checks for infants C1 and C2, and submit one week's worth of sleep logs to LPA Morrison along with a signed and written statement of her understanding that the logs must contain the infants Name, Date and Time of each 15 minute check.
