Based on LPA obervation of two infants in care and the licensee did not furnish evidence to prove that 15-minute check were being conducted for napping infants. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would fill out the sleep log daily for up to ten days for two infants and submit the completed logs to the deparment by 05/25/2022. LPA provided with a blank sleep log and information pertaining to safe sleep.

Based on the Licensee not furnishing a current mandated reporter training certificate. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would complete the online mandated reporter training module at mandatedreporterca.com and Licensee would submit a current certificate to the department by 06/24/2022.

Based on five children's (C1-C5) records were reviewed at 11:00am which revealed C1 through C5 either had an incomplete LIC 700 or Immunization record (IR) was not transferred onto the CDPH 286 form. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain parent signature on the LIC 700 and would tranfer the children's immunization records onto the CDPH 286 form, and would submit the documents to the department by 05/18/2022
