Based on interview and record review, the licensee did not comply with the section cited above in one of out of two staff files. Staff S2 did not have proof of immunity to Measles on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee stated she would ensure that S2 obtained her proof of immunity against the Measles and she would submit S2's required immunization to the Department by 11/26/25 via email. Email: amy.strother@dss.ca.gov
Based on two staff (S1 & LS) records reviewed at 10:45am which revealed S1 did not have proof of immunity against the Measles. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
