Based on the Licensee not furnishing evidence to prove that 15 minutes checks had been conducted for napping infant(s) in care. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated she would document the 15 minutes checks C5's napping on a log, and Licensee would submit one week worth of documentation, as well as a completed LIC 9098 to the Department by 05/16/22 via mail, email or fax. Based on two staff (LS & S1) records reviewed at 9:34am which revealed LS and S1's AB 1207 Mandated Reporter Training certificates. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observations upon arrival of S1 being left alone with two children and S1 did not have a current EMSA approved pediatric CPR/First Aid certification. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five children's (C1-C5) records reviewed at 10:25am which revealed C2 was missing Immunization record (IR). The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five children's (C1-C5) records reviewed at 10:25am which revealed C3-C4 were missing LIC 995 & LIC 627, C1 was missing LIC 9150, and C5's LIC 700 had not been signed by the child’s parent or authorized representative. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
