Based on LPA's observations and the Licensee not furnishing a Sleep Log or evidence to prove that 15-minute checks had been conducted for C2, C3 & C9 during the infants nap time. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated and agreed to document five days worth of 15 minutes checks on the Sleep Logs for C2, C3 & C9, and the Licensee would submit evidence to the Department to prove that 15 minute checks were being conducted for the napping infants by 07/29/2022 via mail, email or fax. Based on nine children's (C1-C9) records reviewed at 10:20am which revealed C3 was missing LIC 9227. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would provide the LIC 9227 form to C3's parent for completion and moving forward, the Licensee would ensure she provided the LIC 9227 to parents for children up to 12 months old.
