Based on LPA's review of three children's (C1-C3) records at 11:30am which revealed that C1-C2 children's records did not contain Immunization Records and transcribed CDPH 286. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will provide all parents a copy LIC995A that acknowledges that the parent or authorized representative has received and read the LIC 995A and keep the bottom signed and dated portion as proof on file. Based on LPA's observation of the children’s outdoor play area which is on a wooden patio deck that had several loose screws & posts that required fortification. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee's statement and review of disaster drill log which indicated that the Licensee did not conduct an emergency disaster drill within the past six months. the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would condcut an emergency disaster drill, document the drill and submit evidence to prove that an emergency disaster drill had been conducted. Based on the Licensee not furnishing evidence to show 15 minute checks had/was conducted for C1 & C4. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would document three days worth on the Sleep Log to prove that 15 minutes checks were being conducted for C1 & C4. Based on LPA's observation of a child under 12 months old napping in a play yard in a bedroom that was on a separate floor than the Licensee. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would produce a written statement to indicate she intends to have the child nap in a play yard on the same floor as the provider. Based on LPA's review on the Licensee's pediatric CPR/First Aid certification which revealed the certification was expired. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
