Based on record review, the licensee did not update the CDPH 286 immunization record forms for children (C4 and C5) and did not possess the immunization record forms for infants (C1 and C2) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would update each CDPH286 immunization record for all children in care and send copies to LPA by email at robert.maciel@dss.ca.gov.
Based on observation, there was no gate preventing access to the bottom of the stairs in the backyard which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would install a gate on the bottom of the staircase and send a picture as proof to LPA by email at robert.maciel@dss.ca.gov.
Based on observation, the door from the daycare room to the laundry room was not locked, allowing access to the laundry room which contained chemicals, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would ensure the door to the laundry room is locked and move all chemicals in the laundry room on the higher shelves, out of reach of children.
Based on record review, the licensee did not record a disaster drill performed within the last 6 months which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she would perform a disaster drill, record it onto the disaster drill log, and send proof to LPA by email at robert.maciel@dss.ca.gov.
Based on observation, two infants (C1 and C2) were sleeping in baby swings and LPA observed C1 and C2 sleeping in the arms of A1 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately placed the infants in their play pens. LPA reviewed the Infant Safe Sleep regulations with the Licensee, who stated that she would submit a written statement to LPA attesting to her understanding of the regulations.
Based on record review, the licensee did not record sleep checks for infants (C1 and C2) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would begin recording the 15-minute sleep checks for all infants in care and send LPA a copy of the sleep log for C1 and C2 from 7/24-7/31 by email at robert.maciel@dss.ca.gov.
Based on the Licensee not furnish evidence to prove she conducted 15 minute checks while C1 napped. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would initiate and document 15 minute checks for C1 on the infant sleep log and Licensee would submit evidence to the Department to prove that 15 minute checks were initiated. Licensee stated she would produce and submit a written plan to show how she intends to comply with the regulation.
Based on the Licensee not furnishing records for C1 & C2 which revealed C1 & C2 were missing LIC 700 & LIC 627. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee not furnishing two of three children's (C1-C3) records at 10:40am which revealed C1 & C2 were missing LIC 995. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
