Based on eight children's (C1-C8) records reviewed at 1:25pm which revealed LIC 627 was missing parent signature for C7 & LIC 700 was missing parent signature for C8. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would provide the LIC 627 and LIC 700 to C7 & C8's parent, obtain signatures on the forms and Licensee would submit evidence of signature on the forms to the Department by 09/12/22 via mail, email or fax. Email: melchisedeck.augustin@dss.ca.gov & Fax: 707-588-5099.

Based on three staff (S1-S2 & LS) records reviewed at 1:51pm which revealed S2 was missing proof of immunity against the Pertussis. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation of a child (C1) under 12 months old napping on a cot on the floor in the Infant Room at 12:51pm. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would review Infant Safe Sleep regulations and Licensee would produce a written plan detailing how she intended to comply with infant Safe Sleep requirements.
