Based on eight children's (C1-C8) records reviewed at 9:31am, which revealed C3 was missing Immunization Record & CDPH 286. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee said she would provide C6's LIC 700 for parent signature, obtain parent signature on LIC 627 for C3, and obtain parent signature on LIC 282 for C4. Based on two staff (LS & S1) records reviewed at 12:34pm which revealed S1 was missing required staff immunization record. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five children's (C1-C5) records reviewed at 1:00pm which revealed C1-C5 were either missing their Immunization Record (IR) and/or IR was not transcribed onto the CDPH 286. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she woule ensure she obtained children's Immunization Records (IR) and transcribe the children's records onto the blue CDPH 286. Licensee intends to submit evidence of completion for POC by 09/27/23 via mail, email or fax.

Based on the Licensee not furnishing evidence to prove she conducted 15 minute checks while C1-C3 napped. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would provide the parents with and obtain the children's completed Individual Infant Sleep Plan (LIC 9227), and Licensee would submit C1-C3's completed LIC 9227 to the Department by 09/27/23 via mail, email or fax.

Based on staff records reviewed at 12:34pm which revealed the Licensee did not have a current AB 1207 Mandated Reporter Training certificate. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would complete the online AB 1207 Mandated Reporter training module at mandatedreporterca.com, and Licensee would submit a copy of her current certificate including the General training certificates to the Department by 10/28/23.
