Based on LPA's observation of the dister drill log which revealed that at least one drill had not been conducted within six months. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee not furnishing evidence to prove 15 minute was conducted for C3. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on six children's (C1-C6) records reviewed at 9:41am which revealed C1 & C3 & C5 were missing Immunization Records (IR). The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
