Based on LPA's observation of an accessible above ground pool that did not meet fencing requirements. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would ensure all doors and windows that lead to the pool yard would be locked and inaccessible, and moving forward, Licensee would ensure she installed permanent fencing to surround the entire pool, and meet fencing requirements.

Based on LPA's observation revealing the facility did not have a functional carbon monoxide detector installed. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated she would replaced the batteries in the detectors and ensure a functional carbon monoxide detector was installed. Licensee intends to submit evidence to show the installation of a functional carbon monoxide detector by 01/19/24.

Based on the Licensee not furnishing current AB 1207 Mandated Reporter Training certificate for herself and S1. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete the online AB 1207 Mandated Reporter Training module at mandatedreporterca.com, and Licensee intends to submit new current certificates for herlself and S1 to the Department by 01/19/24. Emai: melchisedeck.augustin@dss.ca.gov

Based on the Licensee not furnishing a copy of her current EMSA approved pediatric CPR/First Aid certification. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation of an above pool and hot tub installed in the backyard, and Department records confirmed Licensee did not notify the Department of the installation. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated she would produce a written statement detailing she acknowledge and understood of the requirements to report the installation of new projects and bodies of water at the facility. Email: melchisedeck.augustin@dss.ca.gov.

Based on the Licensee's statement confirming she did not have required Immunization Record for herself and S1. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five children's (C1-C5) reviewed at 10:57am which revealed C1 & C2-C5's records were missing Immunization Records. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five children's (C1-C5) reviewed at 10:57am which revealed C1-C5's Immunization Records were not transcribed onto the blue CDPH 286. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
