Based on licensee's statement that she did not have a file for C6 and child file reviews which reveales C6 did not contain a completed LIC 9227. The licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will provide CCL with copies of all required LIC documentation for C6 by 10/26/22.
Based on licensee's statement and inablility to furnish 15-minute sleep logs. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide CCL with a statement acknowledging the requirement to document 15-minute checks for all infants under the age of 24 months. Licensee stated she would provide written statement by 10/26/22.
Based on staff (LS & S1) records reviewed at 1:23pm revealed S1 was missing proof of negative TB clearance, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated proof of negative TB clearance for S1 would be provided to CCL by 10/28/22.
