Based on record review, the director did not comply with the section cited above in identifiers which posess a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, the licensee did not comply with the section cited above in that a change of child care center director that took place in June of 2024 was not reported to the Department within 10 days, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, the licensee did not comply with the section cited above in that 3/5 staff files reviewed (Staff 1 [S1], S4, and S5) did not have proof of renewal of mandated reporter training every two years, which poses/posed a potential health, safety or personal rights risk to persons in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.

California Lead Action Level at Child Care Centers 101700.3 (b)(1): A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: Based on records review of required lead testing, the facility had lead values of 5.5 ppb or above on water outlets: (D)-8 (M)- 8.8. LPA observed both outlets are drinking facutes in classrooms. This is a potential health and safety risk to persons in care.
