Based on record review and interview, the licensee did not comply with the section cited above in that there was not liability insurance, a bond, or a file of affidavits for each child, available to review. Based on interview they have a current liability insurance policy but could not provide documentation of it, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (copies) of 1 of the following: Liability insurance that complies with CCR Section 102417(m)(1), OR a bond that complies with CCR Section 102417(m)(2), OR a file of affidavits (such as LIC282) that complies with CCR Section 102417(m)(3). One of the listed items will be submitted to the Department by the POC due date.

Based on record review licensee did not comply with section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed licensee's MRT was exp. 5/23

Resolution: Licensee stated he will complete the MRT and send a copy of the certificate to CCL by 3/15/24
