Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 adult assisting in the home, interacting with children does not have a criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will contact Guardian between the hours of 8:00am-12:00pm and inquire about the assistants clearance. The assistant present today will not return to care for children until she obtains a criminal record clearance.

Based on record review, the licensees did not comply with the section cited above by completing the mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Training can be found at mandatedreporterca.com. The licensees must complete the mandated reporter training by 5/9/24 and submit to LPA by email or mail.

Based on record review, the licensee did not comply with the section cited above due to her CPR certificate being expired which poses/posed a potential health, safety or personal rights risk to persons in care

Resolution: The Licensee will submit proof of enrollment in an approved CPR/First Aid course, to LPA by email, fax or mail by 5/9/24.

Based on record review, the licensee did not comply with the section cited above due to her assistant present today not having current immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: On or before 5/9/24, licensee will provide LPA Plumboy with the immunizations for her assistant. The immunizations include MMR, TDAP, and INFLUENZA (the influneza may be declined with a wriiten statement). Failure to correct will result in a $100 per day civil penalty until corrected.

Based on record review, the licensee did not comply with the section cited above in 3 out of 8 children's files are missing completed Lic.995A which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall provide all parents a copy of the Notification of Parents' Rights and keep the Acknowledgement of Notification of Parents' Rights with parents' signatures in each child's file. Licensee shall forward copies of Acknoledgement of Notification of Parents' Rights with parents' signatures to LPA by 4/12/24.

Based on observation the licensee did not comply with the section cited above in 1 out of 8 childrens files does not contain Lic. 700 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure C1's emergency information card is signed, and submit copy of CCL by due date of 4/12/24.

Based on record review, the licensee did not comply with the section cited above by not documenting each 15-minute check for infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin using these sleep logs on all infants birth to 24 months, monitoring and recording 15 minute checks and keep the records for 3 years. Each Friday beginning 4/12/24 until 5/3/24, licensee will submit her sleep log to LPA Plumboy.

Based on record review, the licensee did not comply with the section cited above due to her and her assistant Rosie Salavya not having current mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Training can be found at mandatedreporterca.com. The licensee and licensee's assistant must complete the mandated reporter training by 5/6/22 and submit to LPA by email or mail.
