Based on observation, record review, and physical census taken during today's inspection, the licensee did not comply with the section cited above by having 13 children in care (3 infants and 10 preschool age children) which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee is to immediately reduce numbers to within capacity specified on license. Licensee will email LPA Plumboy informing her how she returned to compliance with ratio. LPA to revisit to ensure Licensee is remaining within ratio. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review the licensees husband/Assistant did not comply with the section cited above due to his mandated report training certificate being expired on 1/11/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Training can be found at mandatedreporterca.com. The assistant must complete the mandated reporter training by 02/16/24 and submit to LPA by email or mail. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above in 4 out of 13 children's files did not have immuization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall obtain immunization records for C1, C5, C6 and C7 and submit copy of children's immunization records and cards to LPA by 02/2/2024. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 infants do not have the 15 minute sleep log on file from the day of enrollment which poses a potential health, safety or personal rights risk to persons in care. The licensee only has the month of January on file.

Resolution: Licensee will begin using sleep logs on all infants birth to 24 months, monitoring and recording 15 minute checks and keep the records for 3 years. Each Friday in Feburary 2024, licensee will submit her sleep log to LPA Plumboy.

Based on record review, the licensee did not comply with the section cited above due to the present assistant not having a current mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Training can be found at mandatedreporterca.com. The licensee's assistant must complete the mandated reporter training by 01/07/22 and submit to LPA by email or mail.
