Based on record review of children's files, the licensee did not comply with the section cited above in which C2 , C5 and C6 did not have the LIC627 present in their file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA printed LIC627 for the licensee to have completed by 6/16/2025 COB. Licensee will confirm completion of required forms via email to LPA.

Based on record review, the licensee and assistant M. Kaur did not comply with the section cited above due to their mandated reporter training being expired within the last week which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Training can be found at mandatedreporterca.com. The licensee and assistant must complete the mandated reporter training by 1/31/23 and submit to LPA by email or mail. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Resolution: Licensee shall conduct a fire/disaster drill and submit copy of drill log to CCL by POC date of 02/24/23. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above due to both her assistants present today do not have current immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: On or before 02/28/19, licensee will provide LPA Plumboy with the immunizations for her 2 assistants. The immunizations include MMR, TDAP, and INFLUENZA (the influneza may be declined with a wriiten statement). Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above by not documenting each 15-minute check for infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided sample sleep logs to be used for all infants from age birth to 24 months. Licensee will begin using these sleep logs on all infants birth to 24 months, monitoring and recording 15 minute checks and keep the records for 3 years.

Based on record review, the licensee did not comply with the section cited above in 1 out of 8 files do not have an immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: On or before 1/27/23 licensee shall transfer C5 immunizations onto immunization cards and submit copies to LPA by due date of 10/28/22.

Based on record review, the licensee did not comply with the section cited above due to hert mandated reporter training being expired as well as her assistant present does not have a current mandated reporter training certificate on fiel which poses a potential health, safety or personal rights risk to persons in care.
