Based on record review, the licensee did not comply with the section cited above in that for an 11-month old infant, there was no Individual Infant Sleeping Plan (LIC9227) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have parents fill out and sign Individual Infant Sleeping Plans (LIC9227) for all infants 12 months or younger from now on and going forward and submit a copy of C3s Lic. 9227 to LPA Plumboy. An emailed copy of Lic.9227 was provided to license during today's inspection.

Based on observation the licensee did not comply with the section cited above due to an infant was observed sleeping in a rocker which was placed inside a crib, as well as another infant was sitting in a high chair not eating or drinking.which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed her and her assistant will watch the Personal Rights video online at ccld.ca.gov on or before 02/04/22 and submit a summary to LPA via fax, email and/or mail. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
