Based on record review, the facility did not possess record of the LIC282 Affidavit Regarding Liability Insurance for children (C2-C3) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they would obtain signed LIC282 forms for C2 and C3 and send a copy to LPA by email at robert.maciel@dss.ca.gov by 2/18/26.

Based on observation, there was accessible cleaning chemicals in the cabinet by the stairs in the living room, in the closet by the front door in the living room which were accessible to children in care. Additionally, there was unlocked ammunition in the closet in a bedroom on the second floor which was not accesible due to a child gate at the base of the stairs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they would move the chemicals to an inaccesible location, secure the ammunition in a locked container, and send proof to LPA by email at robert.maciel@dss.ca.gov by 2/18/26.

Based on interview with Licensee, the licensee did not record 15-minute sleep checks for a child (C4) due to believing that they were no longer requred after the child was one year of age which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they would begin recording 15 minute safe sleep checks for C4 and submit a copy of checks performed from 1/21/26 - 1/28/26 to LPA by email at robert.maciel@dss.ca.gov by 2/18/26.

Based on record review, the facility did not possess complete immunization records for children (C2-C5) and did not possess the CDPH286 immunization record form for a child (C1) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they would obtain complete immunization records recorded onto the CDPH286 form for C1-C5 and send a copy to LPA by email at robert.maciel@dss.ca.gov by 2/18/26.

Based on record review, the licensee did not possess record of the Licensee's (S1) proof of immunization against Pertussis or proof of immunization against Measles or Pertussis for the Co-licensee (S2) or the Assistant (S3) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain complete immunization records for S1-S3 and submit a copy to LPA by email at robert.maciel@dss.ca.gov.

Based on interview and record review, the licensee did not record disaster drills performed within the last 6 month which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would record a performed disaster drill and send a copy of the drill log to LPA by email at robert.maciel@dss.ca.gov.

Based on record review, the licensee did not possess a complete LIC9227 Individual Infant Sleeping Plan for child 1 (C1) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain a complete LIC9227 form for C1 and send a copy to LPA at robert.maciel@dss.ca.gov.

Based on record review, the licensee did not possess record of immunizations for child 2, child 3, or child 4 (C2 - C4) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain immunization records for C2-C4 and send a copy to LPA by email at robert.maciel@dss.ca.gov.

Based on observation, LPA observed child 1 (C1) sleeping in a play pen with a blanket, a pillow, and toys which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would submit a statement attesting to her understanding of the safe sleep regulations to LPA by email at robert.maciel@dss.ca.gov.

Based on record review, the licensee has not recorded any 15 minute sleep checks for child 1 (C1) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would begin recording 15-minute sleep checks for all infants in care and submit a copy of the sleep checks for child 1 (C1) performed from 3/5/25 to 3/12/25 to LPA by email at robert.maciel@dss.ca.gov.

Based on record review, the Licensee, Co-Licensee, and Assistant (S1-S3) did not possess a current mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain current mandated reporter training certificates for S1-S3 and submit them to LPA by email at robert.maciel@dss.ca.gov.
