Based on record review, the facility did not possess a complete record of immunizations for child 6 (C6) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would obtain the complete immunization record for C8 and send a copy to LPA by email at robert.maciel@dss.ca.gov by 12/15/25.
Based on record review, the facility did not possess complete LIC9227 Individual Infant Sleeping Plan forms for child 7 and child 8 (C7 & C8) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would obtain the complete LIC9227 Individual Infant Sleeping Plan for C7 and C8 and send a copy to LPA by email at robert.maciel@dss.ca.gov by 12/1/25.
Based on observation, an infant (C8) was dropped off at the facility who was the 5h infant present in the facility which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately called C8's parent who picked up C8 and stated that she would submit a written statement attesting to her understanding of the capacity and ratio regulations, specifically the amount of infants that can be present in the home at any time and send a copy to LPA by email at robert.maciel@dss.ca.gov.
Based on review, staff 1 and staff 2 did not possess current mandated reporter training certificates which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she woud obtain both missing mandated reporter training certificates and send a copy to LPA by email at robert.maciel@dss.ca.gov.
