Based on interview, the licensee stated that she did not record the 15-minute sleep checks for infants in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA explained the requirement to record sleep checks for all infants every 15 minutes while they are asleep. Licensee immediately began recording sleep checks for infants C3 and C5 and stated that she would submit a copy of the sleep checks for C3 and C5 from 10/9/25 - 10/17/25 to LPA by email at robert.maciel@dss.ca.gov.

Based on record review, the child files for C1-C5 did not contain LIC282 affidavit regarding liability insurance forms which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain the missing LIC282 forms for all children in care and send a copy of each to LPA by email at robert.maciel@dss.ca.gov.

Based on observation, the laundry room door was open and cleaning chemicals were accessible on the floor, the child lock for the cabinet under the kitchen sink was unlocked and cleaning chemicals and pesticides were accessible inside, and there was a pesticide container on the ground in the backyard. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Assistant immediately moved the pesticides to the garage which is locked using a padlock and re-secured the child latches for the laundry room door and cabinet under the kitchen sink. Licensee stated that she would develop a plan for reminding herself to check the inaccesible areas for the child proof locks and submit the plan to LPA by email at robert.maciel@dss.ca.gov.
