Based on observation, interview, record review, the licensee did not comply with the section cited above in. Gate did not self-latch and licensee pushed the gate closed with her hand. Licensee stated that she has a key lock on the gate and keeps it locked. Gate did not self-latch which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LIcensee stated she will fix the gate where it self latches.

Resolution: Licensee stated that licensee and assistant will take the Mandated Reporter training A copy ofr the licensee and assistant's current mandated reporter training will be provided to the LPA by email or text by due date of 8/11/23
