Based on interview, the licensee did not comply with the section cited above. Licensee and assistant did not have a current mandated reporter certification available for review during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit mandated reporter certifications for herself and assistant present by POC due date to LPA via text, mail, or email at dean.thompson@dss.ca.gov.

Based on interview and record review, the licensee did not comply with the section cited above. Child (C3) and child (C5) did not have immunization records available for review during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above. During records review LPA was not able to review Diaster Drill Log. Licensee stated she does not have documentation of a disaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will email disaster drill log to LPA by 02/23/2024

Based on record review the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit a proof of children's of immunization by due date via email.

Based on observation and interview, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. Fire Extinguisher indicator points to red which indicates that it is expired or needed service.

Resolution: Licensee agreed to have the fire extinguisher be checked and serviced by due date. Licensee will submit proof via email that fire extinguisher is serviced and should be in green status.

Based on record review,1 infant in care did not have the LIC 9227 available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of completion and provide to LPA via email to mila.quinto@dss.ca.gov by due date.

Resolution: Licensee and assistant will conduct the online training and will submit to LPA via email to mila.quinto@dss.ca.gov by due date.

Based on record review, the licensee's Pediatric CPR/First Aid expired on February 2021. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provid a current CPR certificate and will provide copy to LPA via email to mila.quinto@dss.ca.gov

Based on record review, the licensee and assistant did not have the immunization for measles and pertuses available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of the immunization records for both Licensee and assistant and will maintain a copy for file.
