Based on interview and record review, the licensee did not comply with the section cited above. 1 (one) of 5 (five) records review did not have a health screening on file to reivew which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file reviews, LPA observed staff present did not have proof of their mandated reporter certification available or inside their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above. Lead testing was completed but did not pass the lead testing requirements which poses/posed a potential health, safety or personal rights risk to persons in care.
