Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above. LPA observed in accessible kitchen, knives in the sink, Lysol all-purpose spray and disinfecting wipes underneath the kitchen sink which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA discussed the meaning of accessible and off-limits areas. Licensee stated she there is a mesh safety gate that separates the kitchen area from the living room. LPA informed Licensee, the mesh safety gate MUST be always secured and if the areas are off-limits then no children can be in the area. Licensee stated she understood and will ensure compliance moving forward.
Based on LPA records review and Licensee interview, the licensee did not comply with the section cited above. C1 and C6 are missing LIC700, LIC627, LIC9150, LIC282, LIC995A and immunization records. Furthermore, C2 and C3 are siblings. LPA informed Licensee forms LIC627, LIC9150 and LIC282 MUST be completed for each sibling . The above poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have parents/authorized representatives of each child complete the forms requested and will submit copies to LPA via email by end of business on 01/23/2026.
