Based on Staff Records Review, Staff 4 and Staff 5 are missing personnel records from file (LIC503, LIC9108 and LIC9052). The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, LPA observed multiple play equipment’s (plastic storage bin, play sink and large plastic tub) located in accessible play area with water and soap. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Based on interview with Director Sandra Nunes, the facility's water lead testing was completed in 12/2022. LPA conducted a records review and the following forms were not submitted to Child Care Licensing; LIC9275, LIC9276 and LIC999. Copies of the results were provided to LPA. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on Staff Records Review, S1, S2, S3, S4 and S5's Mandated Reporter Certifications were expired or missing. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
