Based on record review, the licensee did not comply with the section cited above. During today's inspection, Licensee and staff members have not completed renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff member will complete renewal mandated reporter training via www.mandatedreporterca.com and submit certificates to Fresno Community Care Licensing by 12/24/2021.

Based on record review, the licensee did not comply with the section cited above. During today's inspection, a child that was enrolled on 11/15/2021 record did not contain signed LIC 9224 acknowledging the parent/legal guardian received copies of the licensing reports that document the two Type A deficiencies the licensee received during the prior 12-month period which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide parent/legal guardian copies of the two licensing reports as well as LIC 9224 and have parents/authorized representative sign stating they have received documents and the date it was received. Licensee will submit signed LIC 9224 to Fresno Community Care Licensing by 12/24/2021.
