Based on record review, the licensee did not comply with the section cited above. Licensee was unable to produce 3 children's files with a signed LIC 995A which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will give parents LIC 995A and obtain signature and submit copies to Community Care Licensing by 04/29/2022.
Based on observation, and interview, the licensee did not comply with the section cited above. There are locked doors and/or a safety gate which interferes with a child's toileting needs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will not interfere with a child's toileting needs and clear route so a child can freely utilize the bathroom whenever needed and submit pictures to Community Care Licensing by 04/22/2022.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee stated that she did a safe sleep log before but has not completed one for the infants in a long time which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 04/29/2022.
