Based on observation, the licensee did not comply with the section cited above. Licensee had a total of 4 infants and 4 toddlers without an assistant present which posed an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee had a parent pick an infant up. Licensee had an assistant come to assist her putting her back withing capacty. POC cleared today.

Based on record review, the licensee did not comply with the section cited above. Staff member could not provide completed mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have staff member complete mandated reporter training via www.mandatedreporterca.com and submit copy of certificate of completion to Fresno Community Care Licensing by 10/21/2022.

Licensee stated she was unware she had to check sleeping infants every 15 minutes and keep documentation on a safe sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 11/05/2021.

Licensee did not provide a copy of the notice Family Child Care Home Notification of Parent's Rights, LIC 995A (8/06), the Caregiver Background Check Process, LIC 995E (6/05), and the Family Child Care Consumer Awareness Information, LIC 9212 (10/05) to two children's parent or authorized representative which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide the children's parent or authorized representative with a copy of the notice Family Child Care Home Notification of Parent's Rights, LIC 995A (8/06), the Caregiver Background Check Process, LIC 995E (6/05), and the Family Child Care Consumer Awareness Information, LIC 9212 (10/05) and submit copy of signed LIC 995A to Fresno Community Care Licensing by 11/05/2021.

Licensee completed mandated reporter training on 07/01/2019 and has not completed renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
