Based on record review, the licensee did not comply with the section cited above one assistant does not have immunization records on file for influenza, pertussis and measles and one assistant does not have a TB clearance on file for which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will submit copy of immunization record and TB Clearance for assistants to LPA.
Based on record review, the licensee did not comply with the section cited above. Licensee completed initial mandated reporter training on 1/26/2019 and has not completed renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Staff #1 has yet to complete initial mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and and all staff members will complete mandated reporter training via www.mandatedreporterca.com and submit certificate of completion to Fresno Community Care Licensing by 12/03/2021.
