Resolution: Licensee will ensure that immunization records/immunity to MMR for her Assistant is documented for CCL review by poc date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure that she and her Assistant complete mandated reporter training. Training certificates will be kept in staff and Licensee's file. Training will be completed every two years.

Resolution: Licensee shall ensure that all Assistants have the required immunization records on file. Licensee's Assistant shall submit proof of MMR immunization record to CCL by POC date.

Based on record review, the licensee did not comply with the section cited above in 3 out of 8 child files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure that all children have current immunization record on file by the first day of enrollment. Licensee shall conduct an audit of all child files and ensure that all children have current immunization records on file. Licensee shall submit proof to CCL of immunization record for the three children with missing documents by poc date.

Based on record review the licensee did not comply with the section cited above in 3 out of 8 child files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure that all infants under the age of 1 years old enrolled in the childcare program have a completed LIC 9227 form on file. The LIC 9227 shall be completed and placed in the child files. Proof of completion shall be submitted to CCL by POC date.

Based on record review, the licensee did not comply with the section cited above in 3 out of 8 files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will immediately create and document safe sleep logs for infants in care. Licensee shall submit copies of the log to CCL by POC date.
