Based on record review, the licensee did not comply with the section cited above in 2 out of 7 child files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct an audit of all child files and ensure that LIC 282 Affidavit forms are in all child files. Licensee will submit missing LIC 282 forms to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above in one file reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill by POC date and will ensure that fire drills are conducted every six months with enrolled children. Licensee will update fire drill log in accordance to regulation. Copy of current fire drill log will be submitted to CCL.

Resolution: . Licensee will update fire drill log when drills are conducted every six months in accordance to regulation. Copy of current fire drill log will be submitted to CCL.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that her Assistant completes cpr and first aid training by POC date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 7 files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review all child file and ensure that immunization records are in each file. License will ensure that immunization records are received prior to enrollment into the facility. Licensee will provide a copy of the two missing immunization records to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 7 child files reviewed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct an audit on all child files for enrolled children. Licensee will ensure that all files have signed LIC995A forms. Licensee will send a copy of missing LIC 995A forms to CCL by POC date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 infant files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure compliance of all infant safe sleep regulations. Licensee shall create and maintain a sleep log documenting safe sleep checks that are being conducted every 15 minutes. Licensee will review safe sleep regulations and provide a statement acknowledging review of regulation . Licensee will submit a copy the sleep log to CCL by POC date along with the statement acknowledging regulation review.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 infant files reviewed for sleep logs which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall immediately create sleep logs for infants in care. Licensee shall submit a copy of the infant sleep logs by poc date- 6/29/23.
