Based on observation and interview, the licensee did not comply with the section cited above in that she did not have a life ring on the premises, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a life ring and send LPA Beckby a picture of the reciept and life ring that shows it is USCG approved and meets all requirements.

Based on record review, the licensee did not comply with the section cited above in that an LIC9227 form was not completed for 8 month old, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA a copy of a completed and signed LIC9227 by due date.

Based on observation, the licensee did not comply with the section cited above in that she had 3 bottles of sunscreen that were accesible to children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will keep sunscreens out of reach of children by placing them on a top cabinet.

Based on record review, the licensee did not comply with the section cited above in that 2 out 0f 2 Mandated Reporter Certficaites were either expired or not taken, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA copies of Mandated Reporter certificates before due date.

Based on record review, the licensee did not comply with the section cited above in that the roster was not current and did not have children's birthdays, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will update roster completely. LPA will return to verify compliance.

Based on record review, the licensee did not comply with the section cited above in that she only conducted 1 fire drill in 12 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will conduct a fire drill within 30 days and every 6 months after. LPA will return to verify drill was conducted.

Based on record review, the licensee did not comply with the section cited above in that Licensee could not produce an Emergency Disaster Plan, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA a current Emergency Disaster Plan before due date.
