This poses an immediate risk to the health, safety, or personal rights of children in care. Witnesses, Staff #1 & Staff #2 (S1 & S2), revealed that while supervision was in placed, Child #1 (C1) sustained a head injury when he fell backwards from a sitting position (the incident occurred in the playground) due to teacher - child ratio (3 teachers to 13 infants).
