Based on observation, the licensee did not comply with the section cited above in 1 out of 1, lysol on counter at entrance and dining room and shampoo in the tub which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all hazardous items immediately. Licensee will store items in area where inaccessible to children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above licensee in past six months has no documentation of a fire/disaster drill which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete a fire/disaster drill with children in care. Licensee will document drill and submit to LPA for review.

Based on record review, the licensee did not comply with the section cited above licensee has no completed infant sleep logs on file for infants up to 24 months of age which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete infant sleep logs for all infants up to 24 months in age. Licensee will submit infant sleep logs to LPA for review.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above mandated reporter training certificates of completion for licensee and staff are not readily available for review by the department which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete mandated reporter training. Licensee will submit certificates of completion for licenss and staff to LPA for review.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LIC 9052 to staff for signature. Licensee will place completed LIC 9052 in employee file. Licensee will send copy of completed form to LIC for review.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above staff files incomplete as they are missing flu documentation which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that all staff complete flu immunization or flu declination and place in file for review. Licensee will submit completed flu declination or flu immunization record for 2025 to LPA for review.

Based on interview and record review, the licensee did not comply with the section cited above children's file missing children's immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will reequest current immuniztion records from parents for all children enrolled. Licensee will forward children's immunization records to LPA for review.
