Based on observation and interview, the licensee did not comply with the section cited above there was chemicals, cleaning agents, personal hygeine items,and yard tools which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was able to remove most cleaning agents and chemicals in the reach of children; however, there was items left in the backyard that will need to be removed. Licensee will remove all items that are dangerous to child by the date above and will send pictures the cleaned areas to LPA. LPA provided email/ cell phone number

Based on observation and (interview, the licensee did not comply with the section cited above there was a linfer found in the crib as an infant slept, the linfer was also found in the other cribs as well which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA advised licensee that cribs should be free of any items, there should only be a tight fitted sheet in the cribs, Licensee removed the liners from the cribs during the inspection.

Based on interview and record review, the licensee did not comply with the section cited above Assistant present did not have current negative TB test or Immnuzation records present which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA advised licensee to ensure assistant obtains the required immunizations and negative TB test by the date above. LPA provided their email and cell phone number, licensee will send pictures of completed records by the date above.
