Based on records review, the licensee did not comply with the section cited above in that C1 does not have an LIC 9227 completed for C1. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will submit a completed LIC 9227 for C1 to CCLD (shane.loftus@dss.ca.gov) by 01/16/2026.

Based on records review, the licensee did not comply with the section cited above in that C1 does not have required licensing forms on file for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will submit required licensing forms (LIC 700, LIC 627, LIC 995A, and immunization records) for C1 to CCLD (shane.loftus@dss.ca.gov) by 1/16/2026.

Based on records review, the licensee did not comply with the section cited above in that C1 does not have a 15 minute sleep log on file for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will submit a 15 minute sleep log for C1 to CCLD (shane.loftus@dss.ca.gov) by 01/16/2026.

Based on records review, the licensee did not comply with the section cited above in that there is not a fire/disaster drill on file for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will conduct and document a fire/disaster drill and submit proof to CCLD (shane.loftus@dss.ca.gov) by 1/16/2025.

Based on observation and record review, the licensee did not comply with the section cited above by having incomplete children's files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide copies of the completed files to LPA via email by 11/20/2023 at giovani.gonzalez@dss.ca.gov.

Based on observation and record review , the licensee did not comply with the section cited above in 1 out of 1 persons which poses an immediate health, safety or personal rights risk to persons in care.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in two staff were not able to produce a current Pediatric CPR/First Aid card which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will product a current CPR/Aid card to Community Care Licensing or schedule a class no later than 11/22/2021.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in six (6) out of six (6) children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must create files for each of the children enrolled in the daycare no later than 11/22/2021. Licensee advised to submit a written plan of correction (POC) stating measures to be taken in order to follow Title 22 Regulations to prevent the incident from happening again by 11/22/2021.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in five (5) out of six (6) children poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must create files for each of the children enrolled in the daycare no later than 11/22/2021. Licensee advised to submit a written plan of correction (POC) stating measures to be taken in order to follow Title 22 Regulations to prevent the incident from happening again by 11/22/2021.
