FERENCE FAMILY CHILD CARE AKA CREATIVE LITTLE MIND

Based on observation, the licensee did not comply with the section cited above in that there is a pool at the Family Child Care Home that does not have a cover or alarm, a safety ring, a 12-foot pole with a body hook, or daily inspection documentation. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of correction for the above mentioned deficiencies to CCLD (shane.loftus@dss.ca.gov) by 07/31/2025.

Based on observation and interview, the licensee did not comply with the section cited above in that the licensee was not present at the Family Child Care Home during operatoin. Licensee is not scheduled to return until 07/22/2015. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a written plan of correction to CCLD (shane.loftus@dss.ca.gov) explaining that she understands that the regulation was violated and how she will avoid repeating this deficiency from reoccuring in the future.

Based on record review, the licensee did not comply with the section cited above in that both assistants do not have current Mandated Reporter (AB1207) training, this poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of current Mandated Reporter (AB1207) trainings for both Assistants to CCLD (shane.loftus@dss.ca.gov) by 07/31/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. When entering the home at 9:10 AM LPA asked assistant to provide her name. LPA reviewed records and it was determined the assistant was not associated to the facility or been fingerprint cleared. Assistant and licensee stated she has been working at the facility for three months which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed S1 leave the facility during the visit. Licensee agreed to have S1 submit fingerprints before being able to assist in the day care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was reminded in 2018 at last annual visit to complete the mandated reporter training. Licensee did not comply and was unable to provide proof of the mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete Mandated Reporter Training AB1207 by Plan of Correction due date. Licensee was given the website information to complete the training by LPA.
