Based on observation, the licensee did not comply with the section cited above in that, upon inspection LPAs observed that there was no smoke detector in the converted garage "day care room" which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will replace smoke detector and send proof to CCL by POC due date.
Based on record review, the licensee did not comply with the section cited above. Upon record review it was determine that Licensee and Assistant do not have a current Mandated Reporter which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is registered with the Union CCPU-SEIU to take the mandated reporter on 5/15/2023. Licensee will submit proof of training to CCL by POC due date.
Based on record review, the licensee did not comply with the section cited above. Upon record reveiw it was determine that Licensee and Assistant do not have a file at facility with the required documentation. Which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit all required paperwork and corrections by 05/23/2023.
