Based on observation, the licensee did not comply with the section cited above as the fire extinguisher was expired with a service date of 8/2/21 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will either purchase a new fire extinguisher and submit photo proof of fire extinguisher and purchase receipt to LPA by POC due date, or have the existing fire extinguisher serviced and submit photo proof of the updated service tag to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in 3 of the 4 cribs/pack-n-plays to have blankets in place of tight-fitting sheets which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain tight-fitting sheets for the 3 of the 4 cribs/pack-n-plays and submit photo proof to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in 6 out of 6 children's records did not have the LIC 995A which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the parents of each enrolled child sign and complete the LIC 995A and submit photo proof to LPA by POC due date.
