Based on observations made during the inspection. The fact that the fire extinguisher has not been serviced or replaced within the last year is a potential risk to children in care.
Resolution: The Licensee stated she will replace the fire extinguisher and provide proof by the POC date.
Based on record review, the licensee did not comply with the section cited above as the Licensee and Assistant did not have proof of current Mandated Reporter Training available fo review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will complete the Mandated Reporter Training and submit proof of completion to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above as the Licensee and Assistant did not have proof of MMR, Tdap, and Flu shots available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will obtain proof of their MMR, Tdap, and Flu shot records and submit to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in 6 out of 6 children did not have complete LIC 700 forms which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have parents of enrolled children complete the LIC 700 form and provide proof of completed forms to LPA by POC due date.
