Based on observations made during the inspection, the licensee did not comply with the section cited above due to the fact that the fire extinguisher was not serviced in the last year, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will get the fire extinguisher replaced or serviced by the POC date.

Based on observations made during the inspection, the licensee did not comply with the section cited above due to the fact that Child#1 had no sleeping log documented, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will document Child#1's napping time send the proof by the POC date

Based on interview and record review, the licensee did not comply with the section cited above in 2 of 2 staff did not have a current Mandated Reporter Child Care Provider (AB1207) training completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, both staff will complete the Mandated Reporter Child Care Provider (AB1207) training and submit proof of completion by POC date.

Based on record review and interview, the licensee did not comply with the section cited above in 2 of 2 staff did not have immunization records for influenza, pertussis, and measles present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will try to find or obtain immunization records for both staff members and submit proof of immunization for Pertussis, Measles, and influenza (or flu shot declination) by POC date

Based on record review and interview, the licensee did not comply with the section cited above in 3 of 3 child files did not have immunization records present at the facility, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have enrolled paretns obtain their child's immunization records and provide copies to facility. Proof of immunizations will be submitted to LPA by POC date

Based on record review and interview, the licensee did not comply with the section cited above in 2 of 3 child's files were not present at the facility, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have parents complete and obtain all required licensing documents for their childs file. Licensee will submit proof of completion by POC date
