KONG FAMILY CHILD CARE

During file review, LPA observed that the Licensee did not maintain napping log for Child#2 who is under two years old. This poses a potential risk to children in care.

Resolution: Licensee stated that she will begin maintaining the napping log and provide the proof by the POC date.

LPA Lee observed during file review that Child#2 and Child#3 were missing the required LIC 995A form. This poses a potential risk to children in care.

Resolution: Licensee stated she will update the file for Child#2 and #3 and provide proof by the POC date.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 3 children did not have the LIC 700 in their file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee she will request the parents of the child to submit the emergency information card and place in child's file. Licensee will send proof of emergency card to LPA via email.

Based on observation, the licensee did not comply with the section cited above as the carbon monoxide detector's battery was tested to be dead and the carbon monoxide detector was inoperable which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will replace the carbon monoxide detector battery and submit proof of the carbon monoxide detector working to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above as the 2 infants enrolled do not have an Individual Infant Sleeping Plan available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete an Individual Infant Sleeping Plan for each infant enrolled and submit photo proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above as the fire extinguisher's last service date was in March 2021 and the smoke detectors in the home were either missing or inoperable which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will either service the existing fire extinguisher or purchase a new 2A10BC or above fire extinguisher and submit photo proof to LPA by POC due date. Licensee will also install the missing smoke detector in the kitchen and replace the battery in the smoke detector in the play room.

Based on record review, the licensee did not comply with the section cited above as the Licensee did not have current proof of mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the mandated reporter training and submit proof of completion to LPA by POC due date.
