Based on observation, interview and record review, the licensee did not comply with the section cited above where one of three staff were not asswociated. S4 does not have a criminal record clearance on file. Which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of fingerprints for S4. Clearance must be approved before presence in the FCC. A civil penalty of $300 will be assesed for presence February 9-11, 2026.
Based on observation, the licensee did not comply with the section cited above where LPA's observed C3 enter the garage. Garage is an off limits area, where children should not access. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA a written decleration of understanding regarding Title 22 102417 (g). Licensee will also provide proof of garage door lock installed higher to prevent access to daycare children.
Based on observation, interview and record review, the licensee did not comply with the section cited above where licensee was absent 2/9-2/18/26 without CCL notification or emergency closure. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a written decleration of understanding regarding Title 22 102417(a).
Based on interview and record review, the licensee did not comply with the section cited above in one of four staff files reviewed. Where S4 did not have a staff file and immunizations were not available for review during inspection. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a staff file for S4, including proof of immunizations and TB clearance. Staff files must include LIC 9052, Mandated Reporter, LIC 9108.
Based on record review, the licensee did not comply with the section cited above in S3 missing immunizations in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain (S3) Assistants immunizations and submit to LPA via email by 2/23/24.
