Based on observation, interview, record review, the licensee did not comply with the section cited above in that there were 14 children present between the ages of 2years and 4 years, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the 2, the capacity limitations will be followed and the children's schedule will be adjusted so that there are only 12 pre schoolers in care at all times. The license will provide a written schedule and updated Roster vial email by POC due date.
Based on record review, the licensee did not comply with the section cited above in Licensee and Staff #1 did not have a current Mandated Reporter Training Certificarte which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee the Mandater Reporter Training AB1207 will be completed and a copy of the certificate will be sent to LPA by POC Due date.
Based on record review, the licensee did not comply with the section cited above in that the Facility Children's Roster was not updated with the current children's information which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee the Facility Rostar LIC9040 will be completed and a copy will be sent to LPA by POC Due date.
